Allergy/Immunology at National Jewish Hospital and Pediatric Pulmonary Medicine at the University of Colorado, both in Denver.
Publishments/Presentations
Dr. Bukstein is considered a national expert in the use of clinical outcomes in managed care. He has had numerous articles published in such peer-reviewed journals as The American Journal of Managed Care, Allergy, Pediatrics, and Journal of Clinical Outcomes Management. He also gives approximately 50 presentations a year internationally regarding allergic disease, asthma, and quality of life/health outcomes.

Dr. Bukstein has experience as a principal investigator in more than 50 clinical research trials focusing on allergic disease, apnea, asthma, outcomes research, and sinusitis sponsored by such pharmaceutical companies as: GlaxoSmithKline; Merck & Co., Inc.; Aventis Pharmaceuticals; AstraZeneca Pharmaceuticals; Genentech; Novartis; Forest labs; and Respironics HealthScan. He has conducted research sponsored by grants from the National Institute of Health (NtH) and NIAID. He has also been the recipient of clinical medical school grants from Merck & Co., Inc.

Publications/Presentations
Dr. Davis has published numerous manuscripts in well know peer-reviewed journals including the New England Journal of Medicine, Annuals of Allergy Asthma and Immunology, and the Journal of Allergy and Clinical Immunology. He also gives many presentations each year and has been invited to speak at presentations and conferences all over the country. For the past eight years Dr. Davis has been on the Speaker’s Bureau for Merck, Glaxo Smith Kline, Medpointe, AstraZeneca, Aventis, UCB Pharma, Schering, 3M, and Abbot. He leads over fifty conferences per year on asthma and allergies for the past eight years. They have all been by Problem Based Learning (PBL), also known as interactive case discussions. He has been in charge of setting up the PBL faculty and cases to be presented in several national conferences in the last few years.

Dr. Kooistra attended Kalamazoo College (Michigan), and received his MD degree from Michigan State University College of Human Medicine in 1972. His pediatric internship and residency took place at University of Wisconsin Hospitals in Madison where he served as Chief Pediatric Resident. He also completed a two year fellowship in Allergy and Immunology at UW Hospitals.
Professional Activities
Dr. Kooistra, MD, has been practicing Allergy and Immunology in Madison, Wisconsin since 1977. He was also Chief of Pediatrics at Meriter Hospital, Madison. He was Chairman of the Allergy Department of Physicians Plus Medical Group before moving to the Dean Medical Center, where he is also the Chairman of the Allergy Department. He has served on a variety of hospital and clinic committees, and acts as Medical Director of the Madison Neighborhood Asthma Clinic (for underinsured patients). Dr. Kooistra is the founder, President of the Board of Directors and also Medical Director of Camp Wikidas Inc., which is the only camp facility in Wisconsin for children with chronic asthma.
Awards and Recognition
Dr. Kooistra is the recipient of the Distinguished Service Award from the American Lung Association of Wisconsin, and is a member of a number of professional organizations and medical societies. In 2001, he was awarded the Disability Awareness Award from Easter Seals Wisconsin, as well as the Physician Citizen of the Year Award from the State Medical Society of Wisconsin. In 2004, he received a Real Heroes Award from the American Red Cross for his work with the state asthma camp as well as the neighborhood clinic.
